If you need to request a copy of your medical record, you can use MyWellSpan’s Sharing section in the menu and select “Request Medical Records.” Fill out the necessary information to receive your medical record electronically. If you prefer a paper form, use an Authorization to Use or Disclose Health Information.
To make changes to your medical record, MyWellSpan provides an easy-to-use Document Center with Requested Records & Record Amendments. Submit a change request with detailed information or use an online or paper form like the Patient Requested Amendment Form. If it’s for deceased loved ones, use the Next of Kin Verification Form or 21st Century Cures Act Request Form for specific requests related to electronic health information. These forms are designed for different purposes and should be used accordingly.
